Затронуты общие вопросы лечения, включая модификацию образа жизни, поддержание адекватной гидратации и избежание ситуаций, способствующих усилению обструкции. Описана роль традиционно используемых b-адреноблокаторов и недигидропиридиновых блокаторов кальциевых каналов в лечении заболевания. Приведены данные по применению дизопирамида, лечение которым позволяет уменьшить градиент обструкции, выраженность клинических проявлений заболевания и улучшить диастолическую функцию. Особое внимание уделено новому классу препаратов — ингибиторам кардиального миозина, а именно мавакамтену и афикамтену. Сообщаются результаты рандомизированных клинических исследований, посвящённых оценке эффективности этих препаратов. Представлен современный взгляд на место в терапии гипертрофической кардиомиопатии пергексилина, триметазидина, ранолазина, элеклазина, блокаторов рецепторов ангиотензина II, а также комбинации сакубитрила и валсартана.</p></abstract><trans-abstract xml:lang="en"><p>The presented review concerns modern approaches to drug therapy for hypertrophic cardiomyopathy. General issues of the treatment are touched upon, including lifestyle modification, maintaining adequate hydration and avoiding situations that increase obstruction. The role of traditionally used b-blockers and non-dihydropyridine calcium channel blockers in the treatment of the disease is described. Data on the use of disopyramide are presented, the treatment of which allows reducing the obstruction gradient, ameliorating clinical manifestations and improving diastolic function. Special attention is paid to a new class of drugs — cardiac myosin inhibitors: mavacamten and aficamten. The results of randomized clinical trials regarding to evaluating the efficacy of these drugs are reported.
