Их сочетание встречается всё чаще, создавая у пациентов высокий риск осложнений и неблагоприятного исхода. Проведены поиск и анализ 8907 литературных источников из баз данных «Scopus», «Web of Science», «PubMed/MedLine», «The CochraneLibrary» по ключевым словам «diabetes mellitus», «atrial fibrillation», «glycemic control», «hypoglycemic therapy». В обзоре изложены современные представления о механизмах, лежащих в основе развития ФП при СД, а также о влиянии отдельных классов сахароснижающих и других препаратов на риск возникновения ФП, некоторые особенности лечения ФП при её сочетании с СД. </p></abstract><trans-abstract xml:lang="en"><p>Diabetes mellitus (DM) is one of the strongest independent risk factors for the development of atrial fibrillation (AF). Their combination is increasingly common, creating a high risk of complications and poor outcome in patients. A search and analysis of 8907 literature sources from the Scopus, Web of Science, PubMed/MedLine, The CochraneLibrary databases was carried out for the keywords "diabetes mellitus", "atrial fibrillation", "glycemic control", "hypoglycemic therapy". The review presents current ideas about the mechanisms underlying the development of AF in DM, as well as the effect of certain classes of hypoglycemic and other drugs on the risk of AF, some features of the treatment of AF in combination with DM.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>обзор</kwd><kwd>сахарный диабет 2-го типа</kwd><kwd>фибрилляция предсердий</kwd><kwd>сахароснижающая терапия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>review</kwd><kwd>type 2 diabetes</kwd><kwd>atrial fibrillation</kwd><kwd>hypoglycemic therapy</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Исследование не имело спонсорской поддержки.</funding-statement></funding-group></article-meta></front><body><sec><title>Введение</title><p>Сахарный диабет (СД) — одно из наиболее частых хронических заболеваний, выявляемое у каждого десятого взрослого, которое вызывает инвалидизирующие осложнения и сокращает продолжительность жизни людей в глобальном масштабе [<xref ref-type="bibr" rid="cit1">1</xref>]. Заболеваемость СД постоянно увеличивается в связи со старением населения, распространением ожирения, а в последнее время — и под влиянием перенесённого COVID-19 [<xref ref-type="bibr" rid="cit2">2</xref>], причём значительная часть случаев остаётся недиагностированной [<xref ref-type="bibr" rid="cit3">3</xref>]. Под влиянием этих же факторов, а также артериальной гипертензии (АГ), сердечной недостаточности (СН) увеличивается частота выявления фибрилляции предсердий (ФП), регистрируемой примерно у 2% населения в целом [<xref ref-type="bibr" rid="cit4">4</xref>] и также во многих случаях остающейся нераспознанной [<xref ref-type="bibr" rid="cit5">5</xref>]. Примерно у 15% людей с СД имеется ФП, а около 30% случаев ФП отмечается у пациентов с СД [<xref ref-type="bibr" rid="cit6">6</xref>], который является независимым предиктором возникновения этой аритмии, повышает риск её развития почти наполовину. Последнее положение справедливо для СД 1 и 2 типа, а также для преддиабета [<xref ref-type="bibr" rid="cit7">7</xref>].  При сочетании СД и ФП риск сердечно-сосудистой смертности повышается на 25–66% по сравнению с наличием одной из этих патологий, что обусловлено более частым развитием инсульта, прогрессированием хронической болезни почек (ХБП) и СН [<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>Проведены поиск и анализ 8907 литературных источников из баз данных «Scopus», «Web of Science», «PubMed/MedLine», «The CochraneLibrary» по ключевым словам «diabetes mellitus», «atrial fibrillation», «glycemic control», «hypoglycemic therapy». В обзоре изложены современные представления о механизмах, лежащих в основе развития ФП при СД, а также о влиянии отдельных классов сахароснижающих и других препаратов на риск возникновения ФП, некоторые особенности лечения больных с сочетанием СД и ФП. </p></sec><sec><title>Механизмы развития фибрилляции предсердий при сахарном диабете</title><p>Формирование субстрата аритмии в предсердиях больных СД возможно под действием изолированного влияния метаболических факторов на клеточном уровне, но чаще связано с факторами риска/сопутствующими заболеваниями (АГ, СН, ишемическая болезнь сердца), которые также участвуют в аритмогенезе. Хроническая гипергликемия и вариабельность уровня глюкозы в крови, особенно при многолетнем анамнезе СД, являются важными модуляторами ремоделирования предсердий и инициации ФП [<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Ремоделирование предсердий при ФП представляет собой многокомпонентный процесс, который включает электрическую, структурную и вегетативную составляющие. Хроническое субклиническое воспаление и накопление эпикардиального жира, окислительный стресс с увеличением в кровотоке количества активных форм кислорода, характерные для СД, приводят наряду с фиброзом к электромеханическим изменениям в предсердиях — дилатации и снижению их механической функции, которые поддерживают ФП [<xref ref-type="bibr" rid="cit9">9</xref>]. Нарушается связь возбуждения и сокращения предсердий, чему способствуют действующие в качестве паракринных факторов сигнальные молекулы (цитокины, хемокины, адипокины). Электрофизиологическое исследование предсердий выявляет у больных СД укорочение и дисперсию эффективного рефрактерного периода, замедление скорости межпредсердной и внутрипредсердной проводимости, неоднородное пространственное распределение электропроводности [<xref ref-type="bibr" rid="cit10">10</xref>]. Двумя основными признаками структурного ремоделирования предсердий являются дилатация и фиброз. Гипергликемия стимулирует ренин-ангиотензин-альдостероновую систему с экспрессией факторов роста (ангиотензин II, трансформирующий фактор роста-бета1 и другие) и повышением синтеза коллагена. При гипергликемии закономерно увеличивается выработка конечных продуктов гликирования, которые посредством воздействия на специфические рецепторы RAGE активируют фактор роста соединительной ткани и диффузный интерстициальный фиброз, в том числе в левом предсердии (ЛП), что предрасполагает к ФП [<xref ref-type="bibr" rid="cit11">11</xref>]. При СД развиваются гипертрофия и диастолическая дисфункция левого желудочка с увеличением давления его наполнения и дилатацией ЛП, способствующих ФП [<xref ref-type="bibr" rid="cit9">9</xref>]. У больных СД повышаются симпатические и снижаются парасимпатические влияния на сердце, что приводит к укорочению эффективного рефрактерного периода. Неоднородное распределение симпатической иннервации в предсердиях при СД дополнительно способствует их уязвимости к ФП [<xref ref-type="bibr" rid="cit12">12</xref>].</p></sec><sec><title>Влияние уровня гликемии на риск возникновения фибрилляции предсердий</title><p>Согласно результатам метаанализа 34 исследований, повышение уровня глюкозы в крови на 20 мг/дл сопровождалось линейным повышением риска ФП в 1,11 раза (95% доверительный интервал (ДИ): 1,04–1,18) [<xref ref-type="bibr" rid="cit13">13</xref>]. Другой метаанализ, объединивший 14 исследований, показал, что повышение уровня гликированного гемоглобина (HbA1c) на 1% было связано с повышением риска возникновения ФП в 1,13 раза (95% ДИ: 1,08–1,19) или в 1,12 раза (95% ДИ: 1,05–1,20) для пациентов с СД или без известного СД соответственно [<xref ref-type="bibr" rid="cit14">14</xref>]. При этом нельзя исключить возможное влияние абдоминального ожирения [<xref ref-type="bibr" rid="cit15">15</xref>] на наблюдавшуюся связь между СД, уровнем глюкозы в крови и ФП.</p><p>Учитывая важную роль гипергликемии в развитии ремоделирования предсердий, логично предположить, что полное её устранение способно наиболее эффективно предупреждать ФП. Однако интенсивный гликемический контроль не снижал частоту ФП по сравнению со стандартным гликемическим контролем, вероятно, из-за негативных последствий колебаний гликемии, компенсирующих теоретические преимущества интенсивного лечения [<xref ref-type="bibr" rid="cit16">16</xref>]. Периодическая гипогликемия вызывает симпатическую активацию с выбросом катехоламинов и увеличением нагрузки на сердце, повышение уровня высокочувствительного С-реактивного белка и интерлейкина-6, задержку жидкости, которые предрасполагают к развитию ФП посредством активации ренин-ангиотензин-альдостероновой системы и растяжения предсердий [<xref ref-type="bibr" rid="cit17">17</xref>]. По-видимому, более благоприятен стабильный контроль гликемии при СД. На это косвенно указывают результаты когортного исследования у 27 246 пациентов с СД 2 типа, показавшего, что долгосрочная вариабельность гликемии от визита к визиту была независимо связана с развитием впервые возникшей ФП [<xref ref-type="bibr" rid="cit18">18</xref>].</p></sec><sec><title>Сахароснижающая терапия и риск фибрилляции предсердий</title><p>В настоящее время для лечения СД 2 типа применяются сахароснижающие средства 7 основных групп: производные сульфонилмочевины, инсулин, бигуаниды (метформин), тиазолидиндионы, ингибиторы дипептидилпептидазы-4 (иДПП-4), агонисты рецепторов глюкагоноподобного пептида-1 (арГПП-1) и ингибиторы натрий-глюкозного ко-транспортера 2 типа (иНГЛТ-2). Основной целью сахароснижающей терапии СД является поддержание целевого уровня гликемии (HbA1c около 7,0), а также предотвращение колебаний уровня глюкозы в крови, способствующих ремоделированию предсердий и ФП. Учитывая принципиальные различия механизмов действия сахароснижающих препаратов, можно предположить особенности их действия на риск развития ФП [<xref ref-type="bibr" rid="cit17">17</xref>].</p><p>Лечение СД производными сульфонилмочевины и инсулином ассоциировалось с повышенным риском ФП, вероятно вследствие гипогликемии и колебаний гликемии, а также стимуляции симпатической нервной системы, обладающих проаритмическим потенциалом [<xref ref-type="bibr" rid="cit9">9</xref>]. Возможное объяснение связи инсулинотерапии и впервые возникшей ФП также может объясняться неэффективной коррекцией гипергликемии пероральными препаратами до перехода на инсулинотерапию, влиянием значимых сопутствующих заболеваний [<xref ref-type="bibr" rid="cit19">19</xref>].</p><p>В крупном популяционном исследовании (n=645 710) у пациентов с СД 2 типа, не принимавших другие противодиабетические препараты, через 13 лет наблюдения приём метформина после корректировки на влияние сопутствующих заболеваний и лекарственных средств независимо снижал риск впервые возникшей ФП (относительный риск (ОР) 0,81; 95% ДИ: 0,76-0,86; p&lt;0,0001). По мнению авторов работы, отмечавшийся эффект метформина мог быть связан с ослаблением окислительного стресса и миолиза клеток предсердий [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>По результатам метаанализа 3 рандомизированных и 4 наблюдательных исследований с участием 130 854 пациентов с СД, приём тиазолидиндионов ассоциировался со снижением риска впервые возникшей ФП (ОР 0,77; p=0,002) и рецидива ФП (OР 0,41; p=0,002), но только при использовании пиоглитазона (OР 0,56; p=0,04), но не росиглитазона (OР 0,78; p=0,12). Связь между применением тиазолидиндионов и частотой возникновения ФП отмечалась в объединенном анализе 4 наблюдательных исследований (ОР 0,71; р=0,0003), но не была значимой в объединённом анализе 3 рандомизированных контролируемых исследований (ОР 0,77; 95% ДИ: 0,53-1,12; р=0,17) [<xref ref-type="bibr" rid="cit21">21</xref>]. Несмотря на наличие у тиазолидиндионов противовоспалительного и антиоксидантного действия, их способность предупреждать ФП требует подтверждения в крупных рандомизированных исследованиях.</p><p>В ретроспективном исследовании реальной клинической практики у 90 880 больных СД 2 типа, получавших метформин в качестве терапии первой линии, добавление иДПП-4 (чаще ситаглиптина) сопровождалось снижением риска впервые возникшей ФП (ОР 0,65; р&lt;0,0001) по сравнению с добавлением другой терапии (чаще производных сульфонилмочевины) [<xref ref-type="bibr" rid="cit22">22</xref>]. Между тем, по данным метаанализа 6 рандомизированных исследований сердечно-сосудистых исходов у пациентов с СД 2 типа, иДПП-4 существенно не влияли на риск ФП (ОР 0,95; 95% ДИ: 0,78-1,17) и значительно увеличивали риск развития трепетания предсердий (ТП) (ОР 1,52; 95% ДИ: 1,03-2,24) [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>По данным метаанализа 6 рандомизированных исследований оценки сердечно-сосудистых исходов с участием 52 821 пациентов с СД 2 типа, арГПП-1 (альбиглутид, дулаглутид, ликсисенатид, лираглутид, семаглутид и эксенатид) снижали риск смерти от сердечно-сосудистых причин и фатального или нефатального инсульта, но не фатального или нефатального инфаркта миокарда по сравнению с плацебо [<xref ref-type="bibr" rid="cit24">24</xref>]. Однако другие сердечно-сосудистые исходы, такие как частота эпизодов ФП, не были тщательно изучены в этих исследованиях, что объясняет противоречивые данные литературы относительно влияния арГПП-1 на риск ФП. Известные негликемические эффекты арГПП-1 (снижение массы тела и артериального давления, антифибротический, улучшение микроциркуляции, эндотелиальной функции и проведения по миокарду) теоретически способны уменьшить вероятность развития ФП [<xref ref-type="bibr" rid="cit25">25</xref>]. В то же время арГПП-1 повышают частоту сердечных сокращений на 2–8 ударов в минуту по сравнению с контрольной группой, что может быть связано с системной вазодилатацией и последующей рефлекторной тахикардией или прямым влиянием препаратов на вегетативную нервную систему и/или синусовый узел [<xref ref-type="bibr" rid="cit25">25</xref>]. В связи с важной ролью гиперсимпатикотонии в инициировании и поддержании ФП [<xref ref-type="bibr" rid="cit26">26</xref>] этот эффект способен повысить риск ФП. В ретроспективном наблюдательном исследовании у 25 117 больных СД использование арГПП-1 было независимо связано с более высоким риском возникновения ФП (ОР 2,27; 95% ДИ: 1,49-3,47) [<xref ref-type="bibr" rid="cit27">27</xref>]. Эти данные в отношении арГПП-1 не согласуются с результатами метаанализа всех рандомизированных исследований сердечно-сосудистых исходов у СД 2 типа (LEADER, SUSTAIN-6, REWIND, HARMONY, ELIXA и PIONEER), который не показал существенных различий риска случаев ФП между группами арГПП-1 и плацебо (ОР 0,93; 95% ДИ: 0,70-1,23) [<xref ref-type="bibr" rid="cit28">28</xref>]. При этом некоторые метаанализы показали, что применение арГПП-1 по сравнению с другими сахароснижающими препаратами у больных СД 2 типа было связано с более низким риском ФП/ТП [<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit30">30</xref>]. Ограничениями этих исследований является то, что ФП не являлась их заранее заданной и контролируемой конечной точкой. Необходимы дальнейшие проспективные исследования, чтобы подтвердить потенциальное аритмогенное или антиаритмическое влияние арГПП-1 и далее определить, является ли данное свойство специфичным для препарата или класс-эффектом.</p><p>Некоторые плейотропные эффекты иНГЛТ-2 (снижение артериального давления, массы тела, уровня мочевой кислоты в крови, уменьшение объёма эпикардиальной жировой ткани, интерстициального объёма жидкости и дилатации предсердий, повышение уровня магния в сыворотке крови) позволяют предполагать снижение риска развития ФП при их применении [31, 32]. Кроме того, иНГЛТ-2 оказывали прямое воздействие на предсердия, улучшая митохондриальное дыхание за счёт снижения образования активных форм кислорода и потребления аденозинтрифосфата, ингибировали натрий-кальциевый и натрий-водородный обменники в кардиомиоцитах, что сопровождалось уменьшением гипертрофии, ремоделирования, воспаления и фиброза миокарда, улучшением его систолической функции [33, 34]. Наконец, лечение иНГЛТ-2 уменьшало симпатическую стимуляцию [<xref ref-type="bibr" rid="cit35">35</xref>], которая также играет важную роль в развитии и поддержании ФП.</p><p>Ретроспективный анализ результатов исследования DECLARE–TIMI 58 показал снижение риска ФП/ТП (ОР 0,81; 95% ДИ: 0,68-0,95; р=0,009) в группе дапаглифлозина по сравнению с плацебо среди участников с СД 2 типа независимо от установленного атеросклеротического сердечно-сосудистого заболевания, СН или ФП в анамнезе [<xref ref-type="bibr" rid="cit36">36</xref>]. Однако предсердная тахиаритмия не была заранее запланированным контролируемым исходом в этом проекте. В метаанализе 34 исследований (n=63 166) лечения иНГЛТ-2 (дапаглифлозин, канаглифлозин, эмпаглифлозин или эртуглифлозин) отмечалось значительное снижение риска развития ФП/ТП по сравнению с плацебо (ОР 0,81; 95% ДИ: 0,69–0,95; р=0,008) [<xref ref-type="bibr" rid="cit37">37</xref>]. В этой работе только приём дапаглифлозина (10 исследований) был связан со значительно более низким риском ФП/ТП (ОР 0,74; 95% ДИ: 0,60–0,91; р=0,005), тогда как эмпаглифлозин (9 исследований — ОР 1,17; 95% ДИ: 0,75–1,82; p=0,49) и канаглифлозин (8 исследований — ОР 0,81; 95% ДИ: 0,60–1,08; p=0,15) существенно не влияли на риск этих аритмий при сравнении с плацебо [<xref ref-type="bibr" rid="cit37">37</xref>]. В другом метаанализе 20 рандомизированных исследований с участием пациентов с СД, СН или ХБП (n=63 604) применение дапаглифлозина (7 исследований), канаглифлозина (7 исследований), эмпаглифлозина (5 исследований) и эртуглифлозина (1 исследование) с периодом наблюдения от 24 до 202 недель сопровождалось значительным снижением риска возникновения ФП (ОР 0,82; 95% ДИ: 0,72–0,93; р=0,002) по сравнению с контролем [<xref ref-type="bibr" rid="cit38">38</xref>].</p><p>Достаточно неожиданно в исследовании DAPA-HF у пациентов с хронической СН и сниженной фракцией выброса левого желудочка без исходной ФП дапаглифлозин существенно не снижал риск впервые возникшей ФП (ОР 0,86; 95% ДИ: 0,60–1,22) [<xref ref-type="bibr" rid="cit39">39</xref>]. Отсутствие благоприятного эффекта дапаглифлозина на риск впервые возникшей ФП у пациентов с хронической СН и сниженной фракцией выброса левого желудочка в этом исследовании могло быть обусловлено особенностями его дизайна. DAPA-HF являлось исследованием лечения СН, и диагноз ФП ставился на основании анамнеза или однократной 10-секундной регистрации электрокардиограммы при контрольных визитах пациентов. Низкая интенсивность мониторинга ФП могла привести к пропуску эпизодов аритмии, что известно из исследований с использованием расширенного или непрерывного мониторинга ритма сердца [<xref ref-type="bibr" rid="cit40">40</xref>], недооценке всех случаев аритмии у пациентов без ФП в начале исследования, а также переоценке частоты впервые возникшей ФП, которая на самом деле была рецидивом этой аритмии. Рассматриваемый субанализ не имел достаточной статистической мощности из-за малого числа пациентов с впервые возникшей ФП, а период наблюдения (медиана — 18 месяцев) мог оказаться недостаточным для реализации в полной мере кардиопротективных метаболических эффектов ингибиторов иНГЛТ-2 [<xref ref-type="bibr" rid="cit41">41</xref>].</p><p>В отсутствие прямых сравнений в рандомизированных исследованиях современных сахароснижающих препаратов с целью определения их влияния на риск ФП при СД 2 типа представляют интерес результаты крупных исследований реальной клинической практики с использованием статистической методики псевдорандомизации (propensity score matching). При сравнении групп пациентов, получавших иНГЛТ-2 (дапаглифлозин или эмпаглифлозин; n=15 606) или иДПП-4 (чаще линаглиптин, реже алоглиптин, вилдаглиптин, саксаглиптин или ситаглиптин; n=12 383), отмечалось снижение риска впервые возникшей ФП в первой из них (ОР 0,61; 95% ДИ 0,50–0,73; р&lt;0,001) [<xref ref-type="bibr" rid="cit42">42</xref>]. Сопоставление групп больных, принимавших иНГЛТ-2 (дапаглифлозин, эмпаглифлозин, редко канаглифлозин; n=16 566) или арГПП-1 (дулаглутид или лираглутид; n=2746), также показало уменьшение риска впервые возникшей ФП в первой из них (ОР 0,72; 95% ДИ: 0,54–0,97; р=0,028) [<xref ref-type="bibr" rid="cit43">43</xref>]. В ретроспективном когортном исследовании при сравнении 245 442 пар пациентов, получавших иНГЛТ-2 против иДПП-4, 43 682 — иНГЛТ-2 против арГПП-1 и 39 190 — арГПП-1 против иДПП-4 соответственно, иНГЛТ-2 снижали риск впервые возникшей ФП по сравнению с иДПП-4 (ОР 0,90; 95% ДИ: 0,84–0,96; р=0,0028), а также с арГПП-1 (ОР 0,74; 95% ДИ: 0,63–0,88; р=0,0007). Не выявлялось различий в риске возникновения ФП между группами арГПП-1 и иДПП-4 (ОР 1,01; 95% ДИ: 0,86–1,19; р=0,8980). Только дапаглифлозин ассоциировался с более низким риском впервые возникшей ФП по сравнению с иДПП-4 (р=0,02) [<xref ref-type="bibr" rid="cit44">44</xref>]. Все 3 представленных исследования выполнялись с использованием базы данных медицинского страхования Тайваня, не учитывали изменений в медицинском статусе и коррекции лечения во время периода наблюдения, а также типичного для больных СД бессимптомного течения ФП [<xref ref-type="bibr" rid="cit45">45</xref>], что является ограничениями проведенных работ.</p><p>При использовании общенациональных регистров в Дании, Норвегии и Швеции сопоставлялась частота впервые возникшей ФП у 79 343 пациентов, впервые начавших прием иНГЛТ-2 (чаще — дапаглифлозина и эмпаглифлозина, редко — канаглифлозина и эртуглифлозина) и 57 613 новых пользователей арГПП-1. В когорте лечения иНГЛТ-2 риск этой аритмии оказался значимо ниже (ОР 0,89; 95% ДИ: 0,81–0,96) [<xref ref-type="bibr" rid="cit46">46</xref>]. </p><p>Все перечисленные данные требуют осторожной интерпретации и использования только в качестве гипотез возможных будущих проспективных исследований иНГЛТ-2 в качестве средств профилактики и лечения ФП при СД 2 типа, а также СН.</p></sec><sec><title>Другая кардиотропная терапия при сахарном диабете и риск фибрилляции предсердий</title><p>В зависимости от клинической ситуации пациент может принимать препараты для лечения АГ, СН, ХБП и т.д. По данным метаанализа 26 рандомизированных исследований (n=165 387), ингибиторы ангиотензинпревращающего фермента и блокаторы рецепторов ангиотензина II способны предупреждать первый эпизод и рецидив ФП при АГ и СН вследствие уменьшения нейрогормональной активации, ремоделирования левого желудочка, растяжения ЛП, фиброза предсердий и прямых электрофизиологических эффектов (противодействие укорочению рефрактерного периода предсердий и удлинению времени внутрипредсердной проводимости) [<xref ref-type="bibr" rid="cit47">47</xref>]. Нестероидный селективный антагонист минералокортикоидных рецепторов финеренон в исследовании FIDELIO-DKD снижал частоту новых случаев ФП у пациентов с ХБП и СД 2 типа (ОР 0,71; 95% ДИ: 0,53–0,94; p=0,016) [<xref ref-type="bibr" rid="cit48">48</xref>], что может объясняться ингибированием активности альдостерона с предотвращением фиброзного ремоделирования миокарда предсердий [<xref ref-type="bibr" rid="cit49">49</xref>]. Применение антиаритмических препаратов I и III классов для предупреждения рецидива ФП у больных СД проводится согласно действующим рекомендациям с учетом ограничений, возникающих при наличии структурной патологии сердца, СН, ишемической болезни сердца, ХБП [<xref ref-type="bibr" rid="cit6">6</xref>].</p></sec><sec><title>Профилактика тромбоэмболии у пациентов с сахарным диабетом и фибрилляцией предсердий</title><p>Тромбоэмболии, в частности ишемический кардиоэмболический инсульт, являются наиболее тяжёлыми осложнениями ФП. При наличии СД риск тромбоэмболических осложнений ФП возрастает, что объясняют гиперкоагуляцией крови в связи с активацией тромбоцитов, системным неинфекционным воспалением, окислительным стрессом и ослаблением фибринолиза [<xref ref-type="bibr" rid="cit50">50</xref>]. В крупном когортном исследовании (n=37 209) при ФП и длительности СД ≥10 лет частота инсультов оказалась значительно выше (ОР 1,45; 95% ДИ: 1,16–1,82; р=0,001) по сравнению с больными с продолжительностью СД &lt;5 лет [<xref ref-type="bibr" rid="cit51">51</xref>]. При пятилетнем наблюдении за 5363 пациентами с ФП и СД уровням HbA1c &lt;6,5%, 6,6–7,5% и &gt;7,5% соответствовала частота тромбоэмболических осложнений 1,92, 2,66 и 2,74 на 1000 человек/год соответственно [<xref ref-type="bibr" rid="cit52">52</xref>]. В европейском регистре PREFER ФП у больных с ФП и СД, применявших инсулин, риск инсульта/системной эмболии в течение 1 года был значительно повышен по сравнению с ФП и СД без лечения инсулином (ОР 2,96; 95% ДИ: 1,49–5,87; р=0,0019) [<xref ref-type="bibr" rid="cit53">53</xref>].</p><p>При оценке риска инсульта/системной эмболии по шкале CHA2DS2-VASc ≥2 у мужчин и ≥3 у женщин с ФП показано лечение пероральными антикоагулянтами, предпочтительно препаратами прямого действия (апиксабан, дабигатран, ривароксабан). У пациентов с СД и ФП антикоагулянтная терапия должна быть рассмотрена при риске по CHA2DS2-VASc ≥1 у мужчин и ≥2 у женщин с учётом особенностей каждого пациента (большой размер ЛП, высокая нагрузка эпизодами частого ритма предсердий при мониторинге электрокардиограммы по Холтеру, длительный анамнез СД, лечение инсулином) [<xref ref-type="bibr" rid="cit6">6</xref>]. В крупных рандомизированных исследованиях прямых пероральных антикоагулянтов в сравнении с варфарином у больных с ФП (ARISTOTLE, RE-LY, ROCKET-AF, ENGAGE AF-TIMI 48) 20–40% участников страдали СД 2 типа, и результаты их терапии не отличались от итогов этих работ в целом [<xref ref-type="bibr" rid="cit54">54</xref>]. Снижение функции почек, развивающееся у больных СД, требует учёта при выборе прямого перорального антикоагулянта и его дозы [<xref ref-type="bibr" rid="cit55">55</xref>]. Другие вмешательства или хирургические методы профилактики инсульта у больных СД с ФП (ампутация или окклюзия ушка ЛП) не имеют особых показаний по сравнению с рекомендациями у пациентов без СД.</p></sec><sec><title>Катетерная аблация у пациентов с сахарным диабетом и фибрилляцией предсердий</title><p>Раннее восстановление и поддержание синусового ритма у больных с ФП способно снизить риск инсульта и смерти от сердечно-сосудистых причин [<xref ref-type="bibr" rid="cit56">56</xref>]. Для устранения ФП всё чаще применяется катетерная аблация для изоляции устьев легочных вен, которая может применяться после неудачного лечения или непереносимости антиаритмических препаратов I и III классов, а также в качестве терапии первой линии (до фармакотерапии) [<xref ref-type="bibr" rid="cit6">6</xref>]. Среди множества факторов риска рецидива ФП после аблации (значительное увеличение ЛП, длительный анамнез ФП, пожилой возраст пациента, АГ, снижение функции почек, распространённый фиброз предсердий по данным магнитно-резонансной томографии) присутствует и СД [<xref ref-type="bibr" rid="cit9">9</xref>]. В European Observational Multicentre Study проанализированы результаты катетерной аблации ФП у 2504 пациентов (9% участников с СД). Процедурные осложнения (тампонада перикарда, инсульт, кровотечение, паралич диафрагмы и т.д.) встречались с аналогичной частотой у пациентов с СД и без диабета. При многофакторном анализе СД 2 типа оказался независимым предиктором рецидива ФП через год (ОР 1,39; 95% ДИ: 1,07–1,88; р=0,016), включая эпизоды бессимптомной ФП [<xref ref-type="bibr" rid="cit57">57</xref>]. В этой связи представляют интерес результаты рандомизированного исследования у пациентов с СД 2 типа, у которых частота рецидива ФП в течение 12 месяцев после катетерной аблации оказалась значимо ниже в группе лечения иНГЛТ-2 тофоглифлозином по сравнению с иДПП-4 анаглиптином (24% против 47% случаев соответственно; р=0,0417) [<xref ref-type="bibr" rid="cit58">58</xref>].</p></sec><sec><title>Заключение и выводы для практикующих врачей</title><p>Сочетание ФП и СД встречается всё чаще, ассоциируется с повышенной заболеваемостью (сердечная недостаточность, инсульт) и смертностью. Гипергликемия, выраженная через уровень HbA1c, и способы её коррекции существенно влияют на риск развития ФП. Имеющиеся в настоящее время данные позволяют предполагать преимущество иНГЛТ-2 в сравнении с другой сахароснижающей терапией в отношении предупреждения ФП. Антикоагулянтная терапия, лечение самой ФП у больных СД должны проводиться с учётом влияния этой эндокринной патологии на систему гемостаза, функции сердца и почек пациентов.</p><p>Конфликт интересов.
